How it works
Secure online registration
Our office will send you an email with instructions. You will need this email to access and securely fill out all the necessary forms from your computer or handheld device under the online registration section. Once filled out, you can submit them right there, and you will have an opportunity to keep a copy of your completed documents for your records.
If you are not clear about any question, please leave it blank. You will have a chance to clarify it at the office when you come in. Please note the informed consent form is for your information only, and you will sign it in the office.
